Ind., Local Rule B-2016-1(c)(2)(A): the streamlined fee is available only if "Counsel has filed an executed copy of the “Rights and Responsibilities of Chapter 13 Debtors and Their Attorneys,”" which says the attorney shall "Object to improper or invalid claims, if necessary, based upon documentation provided by the debtor or trustee."
